When visiting DEN MASAFUMI DENTAL CLINIC for the first time, please bring your travel insurance form and an identification document (such as a passport or driver’s license).
Please note that our clinic operates by appointment only, so you must make a reservation in advance by phone.

＊If you have any medication allergies, please inform us in advance.
＊In Japan, medical treatments and procedures are not subject to consumption tax. However, a 10% consumption tax applies to purchases such as toothbrushes and the issuance of medical certificates for treatments.
